| OT/PT Request for Consultation Process |

Prior to filling out a Request for OT or PT Professional Consultation, the teacher and/or educational team should try basic interventions and record data on the results of the interventions. If the inventions are not successful, then a "Request for Occupational or Physical Therapy Professional Consultation for Problem Solving" form should be completed (front and back sides). The form should then be sent to the OT/PT Department at the WHS-Washington campus via the district's mail. Please only place one discipline per request form. If you want to make a request to both OT and PT, then please complete two separate forms (1 for OT and 1 for PT). The new OT Resource Guide can be found on Inside Edition under the "Forms" section then in the "group" drop down select "OT/PT Professional Consultation Process" from the menu. You should now see the Resource guide and the above Request for Consultation forms. |
